Scioto County has two vital-statistics counters, and they do not keep the same deaths.
The county health department is at 612 6th Street, Suite D, Portsmouth, 45662. Phone (740) 355-8358, option 1. Counter hours are Monday through Friday, 8:30 a.m. to 4:15 p.m. A certified copy is $25. The office takes cash, a money order, a cashier’s check, or a local check to the Scioto County Health Department. Do not mail cash. Mail requests need the form, payment, a phone number, and a stamped return envelope.
That office holds Ohio birth records from December 20, 1908 forward. Death records run from 1931, and only if the death was in Scioto County outside Portsmouth city limits. A death inside the city is not their file.
Portsmouth City Health Department is the other counter, at 605 Washington Street. Phone (740) 353-5153. The city has said walk-ins can be same-day, at $25 a certificate, cash or a check with a photo ID, and no debit or credit cards. Mail orders there are a money order to the Portsmouth City Health Department, with a turnaround of about one to two weeks, and hours listed at 8 a.m. to 4:30 p.m.
A birth outside Ohio is that state’s vital records, not either Portsmouth desk. Fetal-death and heirloom certificates go through the Ohio Bureau of Vital Statistics.
Match the death to the city limit before you pick a line. A county clerk cannot print a Portsmouth death, and the bank can only take a certified copy.
