The paper in the baby book is not the paper the BMV wants. Ross County Health District’s vital statistics office issues the certified copy, the one with the seal. The office is at 150 E. Second Street, Chillicothe, 45601. Phone is (740) 779-9630. Email is vitals@rosscountyhealth.org. Counter hours are Monday through Friday, 8 a.m. to 4 p.m., holidays off.
A certified birth certificate or death certificate is $30 a copy. Burial permits are $10. Cash, card, or money order. The district stopped taking personal checks for these records in 2020. Mailed requests are listed as a one-to-two-business-day process once they have the application and the fee. Online orders run through the district’s ordering portal and add a nonrefundable service fee, about $5, plus shipping if you are not picking up.
The office keeps birth and death records for events in Ross County. The district’s own pages put routine certificates from the late 1920s forward at this counter, and older Ross County birth records with the records center. A birth in another county is that county’s job. A birth in Kentucky is Kentucky’s job. Calling Chillicothe for a Circleville birth wastes the stamp.
This is the document that unsticks a Real ID visit. The BMV wants an original or certified copy from vital records, not a hospital souvenir and not a phone photo. If the name changed at marriage, the birth certificate alone still will not match the license. That second paper comes from probate, not from the health district.
Order the copy before the appointment. A same-morning walk-in can work if the clerk is free. It is a bad plan if you are due at the deputy registrar at 9.
If you only need one thing from this: $30, Second Street, certified copy, and do it before the BMV date. The hospital folder stays in the drawer.
