$240 Million Medicaid Home Health Payouts in Two Franklin County Zip Codes Spark Auditor Fraud Probe

COLUMBUS, Ohio. Ohio State Auditor Keith Faber has raised serious concerns about potential fraud, waste, and abuse in the state’s Medicaid home health care program. Two Franklin County zip codes stand out for drawing an outsized share of spending.

Auditor Faber told legislative committees in March 2026 that Ohio directed roughly 1.6 billion dollars toward home health services statewide. Franklin County, home to about 11.5 percent of the state’s population, received 38 percent of those funds. Within that county, zip codes 43229 and 43231 together accounted for approximately 240 million dollars. Auditors called the concentration statistically unusual.

The two zip codes sit roughly four miles apart on the north side of Columbus. Public records show hundreds of Medicaid registered home health providers linked to a small number of buildings along East Dublin Granville Road. Reports have cited as many as 288 companies associated with just seven structures in the area. Many bill for services such as companionship and personal care. These services occur in private homes and prove difficult to verify without strong oversight tools.

Auditor Faber’s office has conducted compliance reviews of individual Franklin County providers. These reviews have uncovered improper payments totaling thousands to hundreds of thousands of dollars. Issues include missing documentation and unauthorized services. The state’s single audit for fiscal year 2025 reported a 15.6 percent error rate in tested Medicaid eligibility samples. That includes payments for ineligible recipients such as deceased individuals. If the rate holds across the full program, questioned costs could reach billions.

Ohio Department of Medicaid officials say they maintain internal controls and work with the Auditor and Attorney General on fraud prevention. They note some referenced providers no longer participate in the program. Investigations continue.

South central Ohio taxpayers help fund Medicaid across the state. Any improper spending diverts resources from legitimate care needs. Auditor Faber encourages residents to report suspected fraud through official channels at the Ohio Auditor of State website.
